Palliative Care - Children’s Centre At Dorset County Hospital

Palliative Care services are provided in partnership with Poole Hospital, the local hospices (Naomi House, Julia’s House) and the palliative care team at Southampton Hospital. Service users must be registered with a Dorset GP, but out of area referrals are considered on a case by case basis. The paediatric team have a lead Paediatrician and Nurse for palliative care and in addition palliative care is part of the core Children Community Nurse Service. Care starts at diagnosis and is provided by the multi-disciplinary care team depending on individual needs for example medical and nursing care, physiotherapy, occupational therapy, and dietician. The core team is supported by the Children Community Nurse service and wider Paediatric Medical and Nursing Teams. Children and families are supported to develop an advanced care plan for their child and at end of life are given choices regarding where they wish to have an individual care plan which can include a 24/7 service at the end of life.
